Parental Consent Form

Parents and/or Legal Guardians please review this Parental Consent Form and the Official True Teryn – Princess Lilla Costume Competition Contest (the “Contest”) and your signed consent is now required in order for your child to participate and receive his/her prize if selected. If you agree that your child may participate in the Contest pursuant to the official giveaway/contest rules and the terms of this Parental Consent Form, then please complete and sign the form below.

Submissions that are not accompanied by a properly signed and completed Parent Consent Form and any other necessary information or materials will not be accepted by Lilac Grove Entertainment, LLC (“Company”). Note: contest entrants who are 18 or older may complete and sign the Entry Form themselves. Residents of Alabama and Nebraska must be 19; Mississippi residents must be 21

By signing this Parental Consent Form, you agree to the following:

1. I hereby agree that my minor child may participate in the Contest being conducted by Company, and irrevocably grant my permission and authorize Company to record, re- record and photograph my minor child’s likeness, name, performance, voice, and interviews of my child, for purposes of the Contest, and for in advertising, marketing, publicity and promotion related to the Contest, and to reproduce and publish the same throughout the universe in perpetuity, in any and all media now known or hereinafter devised. I further agree that the Contest, my minor child’s participation in the Contest and all of the Contest elements, including all recordings, footage and still photographs taken or submitted in connection with it, belong entirely and exclusively to Company, so that Company and its licensees and assigns may duplicate, distribute, broadcast, exhibit, or otherwise use the Contest, its elements, or related still photographs and/or video recordings, without limitation or restriction throughout the universe in perpetuity. Company may edit and adapt my child’s participation as it deems appropriate.
2. I understand and agree that neither I nor my minor child will own any rights in the Contest or the Contest elements. I am granting Company a free, non-exclusive, irrevocable license to use all of the recordings and photographs of my minor child on film, videotape, audiotape, or otherwise as Company may desire. I acknowledge that Company shall have no obligation at all to use my minor child’s name, likeness, interviews of my minor child, or refer to my minor child in the Contest or the Contest elements.
3. I agree that neither I nor my minor child shall make any claims against Company based upon distribution or exploitation of the material or the Contest and its elements to which I have consented on behalf of my minor child. I release Company and its licensees and assigns from any and all claims and liabilities whatsoever that may occur in relation to or arising from my minor child’s participation in the Contest, from recording, filming or videotaping my minor child, and from showing, using or distributing such recordings and photographs. I agree that this Consent and Release will be governed by and construed in accordance with the laws of the State of California.
4. If my minor child’s participation includes music performed, owned, composed, or arranged by my minor child, I hereby grant a free, non-exclusive, irrevocable license for synchronization and performing rights to Company so that the music can be used in the Contest and related distribution. If my minor child’s participation includes still photographs, footage, artwork or other materials owned by my minor child, I grant to Company a free, non-exclusive, irrevocable license to use such materials in the Contest and related distribution.
5. I confirm that I and my minor child live in the United States of America. I also confirm that we live in a state/country where my child at his or her current age is legally allowed to participate in contests with parental consent
6. I agree that my minor child may participate in the Contest described in the Official Contest Rules and is permitted to accept the prizes awarded to the winners.
7. I understand my minor child’s name and other information that I provide on behalf of or related to my minor child, as described in the Official Contest Rules, may be posted together with my minor child’s submission upon Company’s website or other public forum without any additional notice to, or permission by, me. I understand my minor child may participate in any media or promotional events, including any possible winner ceremonies, media interviews, and publicity events, related to the Contest.
8. I hereby warrant that I am a legally competent adult and a parent or legally appointed guardian of the minor child named above, and that I have every right to contract for the minor child in the above regard.
9. I certify and acknowledge that I have read, understood and agree to the Terms and Conditions set forth in the Official Contest Rules, and that I have read this Parental Consent Form prior to signing it; that I have the full authority to give my consent and release so that my minor child may participate in the Company’s Contest and that I consent to provide the information concerning my minor child that is necessary to enter the contest and to receive the prizes to be awarded.

Privacy Disclaimer: Any personal information collected during the course of the Contest by Company will be used for administering this competition, or as otherwise set out in the Official Contest Rules. Except where prohibited by law, participation in the Contest constitutes the entrant’s and his or her parent’s/legal guardian’s consent to the storage, use and disclosure of the entrant’s entry details as set out in the Official Contest Rules.

Accordingly, and intending to be legally bound, I voluntarily agree to the terms set forth herein and acknowledge and agree that my minor child and myself are bound by these Official Contest Rules and I am bound by the terms of this Parental Consent. This release shall be binding upon my minor child, me, and our respective heirs, legal representatives, and assigns.
